The New Patient Welcome Packet explains
who we are and why we need the HIPAA
AUTHORIZATION FORM B completed.
These are three forms that every returning patient must complete before seeing Dr.
Woodcock.
The first is a Symptoms Checklist.
Mark those symptoms you are experiencing leave it blank if you are not.
The second is a Medication List.
Please list all medications, vitamins, and supplements that you take. This
form is required at every visit so we suggest you keep a log and just bring in
your updated log at every visit. On the back you'll find an allergy form.
Let us know what you are allergic to including non-medication items. IE:
hay fever, cats, etc.
The third is a Problem List.
This form is to help you remember issues to discuss at your appointment. At the bottom please list those healthcare professionals you
are currently seeing whether or not they apply to why you are seeing us. Please list all therapists too.
These forms help expedite and complete your appointment in our office.
If you are experiencing any of the following symptoms please complete the
appropriate form.
If you are experiencing bodily aches and pain please complete the
Pain Assessment Questionnaire. This form
lets us know where your pain is located, the degree of pain you are feeling,
and the medications past and current you are taking for this pain.
If you are experiencing headaches or migraine pain please complete the
Headache Questionnaire.
Referring Your Client to Our Practice
Thank you for referring your patient/client to our practice. Attached is paperwork
required to initiate your client's appointment. To expedite the appointment
process please follow these steps.
1. Call our office and speak with the New Patient Coordinator or Sara Burns to schedule your
client.
2. The New Patient Referral Worksheet
must be completed prior to your client's appointment.
3. Your client is required to complete our New Patient Packet
before their appointment. It is the patient's responsibility to
provide medical records prior to their appointment date. They will need to provide
current proof of insurance at this
and all visits. Having these items completed and available will expedite their
appointment.
